Dr. Marsh Cuttino has partnered with Rockwell Physicians to provide aviation physicals. Dr. Cuttino is certified by the FAA to conduct Class 2 and Class 3 FAA Aviation Medical Examinations.

Before the FAA Exam:

Complete items 1-20 of the FAA Form 8500-8 through the FAA MedXPress (MedX) system. After completion, there will be a confirmation number and instructions to print a summary sheet. You must use your home address when completing this form. The data entered through the MedXPress system will remain valid for 60 days. The medical exam cannot be done until this step has been completed.

What to bring with you:

  • MedX confirmation number
  • A form of government-issued identification. All applicants must show proof of age and identity under 14 CFR §67.4 section A. This form must be a government-issued photo identification (e.g., driver’s license, identification card issued by a driver’s license authority, a military identification, or passport.) Applicants may use other government-issued identification for age, such as a certified copy of a birth certificate, however, separate photo identification (such as a work badge) is required.
  • Class 3 Exams ($200), Class 2 Exams ($250), or Class 1 ($300). Payments may be made by cash, check or credit card; there is a small convenience fee for paying by credit card.  EKGs may be extra cost if required.
